Complete our short referral form with basic details about the young person and the support being considered.

Our team reviews the referral to consider needs, location, hours and whether we can provide the right support.

If suitable, we gather further information and documents to plan safe and effective provision.

We agree a personalised package of support based on needs and intended outcomes.

We carefully introduce provision, building trust, and relationships first, before progressing at the right pace.

Who Can Make a Referral?

Is AC Group Education Suitable?

✓ Has SEND or SEMH needs
✓ Experience anxiety or barrier to attending school.
✓ Are experiencing EBSA
✓ Need a smaller, personalised learning environment
✓ Have struggled to engage with previous provision
✓ Require 1:1 or community-based support
✓ Need help developing confidence and life skills.
✓ Are working towards Functional Skills or academic outcomes
✓ Need support preparing for adulthood or future pathways.
